Collaborative Management Programs (CMP)

Hilltop's Collaborative Management Programs help local agencies work together to better support children, youth, and families who are involved in more than one system (like schools, child welfare, or mental health services). The goal is to make services more connected, effective, and efficient. To refer someone to CMP, or learn more information, please use the forms and contact information below.
